Diabetes Prevention Program (DPP)

Diabetes Prevention Program (National DPP) is proven to prevent or delay the onset of type 2 diabetes. Reducing 5% of body weight through sustainable lifestyle change cuts the risk of developing type 2 diabetes by half. HabitNu program is fully recognized by the Centers for Disease Control (CDC) based on its outcomes. HabitNu provides both online and in-person programs.


Diabetes Self Management 

Blood glucose monitoring and diabetes management education can improve health outcomes in people with diabetes, including better blood glucose control. HabitNu’s diabetes care program is structured similar to the proven diabetes prevention program. Education and monitoring are provided by diabetes educators and lifestyle coaches.

Hypertension increases the risk for heart disease and stroke, therefore it is important to keep the blood pressure normal. HabitNu’s heart health program provides remote monitoring of blood pressure levels. HabitNu heart health is a structured, coaching-based program provided by a care team of health care professionals and lifestyle coaches.

Behavioral weight loss is a sustainable program built on intense behavioral counseling and nutrition management. HabitNu Weight Loss program includes education for skill development, strategies to enhance motivation toward goals, and identification of environmental and social support.
