This Privacy Policy is designed to inform users of Prana Diabetes Inc., d/b/a HabitNu, a Delaware corporation (“HabitNu”) website, software application, related solutions, and services (collectively, “Software”) about how we gather and use personal information collected by us in connection with your use of the Software. You may have been invited to use, download or install the Software by HabitNu or a third-party as part of a lifestyle change or clinical care initiative. This Privacy Policy covers how HabitNu treats your Personally Identifiable Information (“PII”) and Protected Health Information (“PHI”) (PII and PHI, collectively hereafter “Personal Information”) that HabitNu collects, receives, maintains, stores, or transmits, including information you transmit or submit to the Software. Your Personal Information includes information that individually identifies you or is information about you that can be traced back to you, your IP address, or your location. It may include, but is not limited to your name, address, employer, email address, phone number, and information about your health. Please note that by using or accessing the Software, you are accepting the practices described in this Privacy Policy. If you do not agree to all of the terms of this Privacy Policy, please do not use the Software. Please read this Privacy Policy carefully.
CHANGES TO THIS PRIVACY POLICY
We may update this Privacy Policy from time to time by posting a new version online. You should check this page occasionally to review any changes. Your continued use of the Software after any modification to this Privacy Policy will constitute your acceptance of such modification.
INFORMATION COLLECTION & USE
HabitNu collects Personal Information when you register with HabitNu or when you use HabitNu’s Software. When registering with HabitNu, we ask you to provide us with your name, address, e-mail address, phone number, sponsoring provider and monitored health data for which you are registering and other related information that HabitNu may ask of you from time to time, and you may submit additional information and comments through the Software.

We may use your contact information to send you information about the Software or our other services we may provide to you, or information relating to your health. It is always your choice whether or not to provide us with such information. If you choose not to provide the requested information you may not be able to use certain features or functions of the Software or services. Whenever you visit our website, HabitNu also receives and records information on our server logs from your browser, including your IP address, HabitNu’s cookie information, and the pages you request, and relates it to the Personal Information you provide.
HabitNu collects and uses Personal Information you provide for the following purposes:
  1. Registration: When you register for the Software, we collect your Personal Information as part of the registration process, including but not limited to your name, address, location, sponsoring provider, and program preference.
  2. Self-Reported Health-Related Information: We collect the information that you enter during the course of using the Software such as information regarding your health and related behaviors.
  3. Provider or Device-Reported Health-Related Information: We collect the information about you that is submitted with your permission by your health care provider or health monitoring device during the course of using the Software, such as information regarding your health activities and/or medical condition, including information that is protected under the HIPAA privacy and security regulations.
  4. Social Information: We collect information that you provide to us pertaining to the people with whom you consent to share your Personal Information (such as a family member, another patient or doctor).
  5. Communications with a Health Professional: We collect communications that take place through the Software between you and your healthcare provider, health manager or lifestyle coach.
  6. Public Forums: We collect information that you publicly post through the Software on our community chat portal.
  7. Email: If you communicate with us via email, then we will collect your email address.
  8. Automatically Tracked Health-Related Information: We may use automated tracking methods such as cookies and connected accelerometers, to collect information regarding your behaviors relative to the Software.
  9. Demographic Information: We may also collect demographic information, such as age, gender, and geographic location, as part of your profile in the Software.
  10. Surveys and User Research: From time to time, we may send you survey questions or contact you with questions related to your experience to provide us with feedback on our Software. We collect any responses that you provide. Participation in surveys or research tasks is elective and does not necessarily impact your access to the Software.
DATA RETENTION
This includes Personal Information such as your registration details, self-reported health-related information, provider- or device-reported health-related information, communications with a health professional, and automatically tracked health-related information, all as described above. We retain this information to support continuity of care, enable you to resume the Software if you choose to do so, maintain accurate records for your sponsoring provider where applicable, and comply with our legal and regulatory obligations.
HabitNu retains your Personal Information for as long as your account remains active in order to provide you with the Software and related services. We retain this information indefinitely unless and until you request that it be deleted, as described below, or unless a longer retention period is required by law (for example, certain health records associated with insurance billing or clinical care may be subject to minimum retention periods under HIPAA or applicable state law, even after account closure).
DATA DELETION
You may request deletion of your Personal Information at any time by using the deletion option available within the Software's account settings. Upon receiving a deletion request, we will delete or de-identify your Personal Information, except where we are required or permitted to retain certain information to comply with legal obligations, resolve disputes, enforce our agreements, or as otherwise required under HIPAA or applicable state law for records tied to billing or clinical care. Where Personal Information is retained for these limited purposes, it will no longer be used for any other purpose.

AI ASSISTANT CONSENT
Purpose
HabitNu's AI Assistant uses artificial intelligence (AI) to provide health information, educational content, and personalized wellness recommendations.
Information Shared
When you use the AI Assistant, HabitNu securely transmits the following information to Amazon Web Services (AWS) for processing using Amazon Bedrock:
  1. The messages, questions, and prompts you enter.
  2. Health information you choose to share, including information such as glucose readings, food logs, weight, activity, medications, symptoms, and program progress.
  3. Other information necessary to generate an AI response.
How Your Information Is Used
The information described above is transmitted solely to generate AI responses to your requests through the AI Assistant. It is processed securely using Amazon Bedrock and handled in accordance with HabitNu's Privacy Policy. HabitNu does not use this information to train public AI models.
Your Consent
By selecting Continue, you acknowledge that you have read this notice and consent to HabitNu transmitting the information described above to Amazon Web Services (AWS) for processing by Amazon Bedrock to generate AI-powered responses.
