HabitNu, a CDC-recognized provider of coach-led lifestyle and behavior change programs and a national Umbrella Hub for the Diabetes Prevention Program (DPP), has partnered with Fasten Health to support identity verification, eligibility confirmation, and patient-authorized health data access as part of CMS ACCESS workflows.

Through this integration, members can verify their identity using CLEAR via Fasten Health, confirm eligibility for covered benefits like MDPP, and securely connect their electronic health records. Clinical data, including labs, diagnoses, and medications, becomes available within the HabitNu platform to support enrollment and program placement.

This integration helps close the gap between eligibility and participation. By bringing identity, eligibility, and clinical data together at the start, HabitNu makes it easier for members to enroll and stay engaged over time, improving enrollment, retention, and completion rates.

The Medicare Diabetes Prevention Program (MDPP) is a fully covered benefit. However, for payers and health systems, the challenge is conversion and follow-through. Eligible members are identified, but too few enroll, and fewer complete the program.

HabitNu is built to address this gap.

Once enrolled, members participate in the CDC-recognized MDPP through small group cohorts, ongoing coach support, and simple daily tracking. Coaches monitor participation and intervene early when engagement drops, helping more members stay in the program and complete it.
