CHARLESTON - HabitNu’s Class Finder tool is now helping state health departments across the U.S. connect thousands of at-risk residents to qualified Diabetes Prevention Program (DPP) providers, both inside and outside the HabitNu Umbrella Hub.
Developed in close collaboration with public health teams, the platform simplifies the participant journey from awareness to enrollment. Residents can explore a statewide map of all CDC-recognized DPP providers, review current and upcoming classes, take a quick risk assessment, and enroll in the program that fits them best, virtual or in-person.
Once a resident signs up, their information is securely routed to the selected provider. Organizations in the HabitNu Umbrella Hub see the participant directly in their platform cohort and can begin outreach immediately. Providers outside the Hub access referrals through a secure referral portal that includes task tracking and reporting tools, ensuring no eligible participant is left behind.
State health departments can embed the Class Finder directly into websites, landing pages, and campaigns, making it easy for them to plug into existing diabetes prevention and awareness efforts.
Missouri’s Department of Health and Senior Services has integrated the Class Finder into its “
Take Five Steps” diabetes prevention campaign. Residents who respond to DPP marketing campaigns online are routed straight to the platform, where they can check eligibility and join a DPP class near them.
“We kept hearing the same frustration, people were ready to join a DPP class but didn’t know where to look or who to call,” said Dr. Sindhu Rajan, CEO of HabitNu. “Now they can see every available class across the state in one place. It’s simple, intuitive, and built to make enrollment effortless, whether the provider is inside our Umbrella Hub or not.”
The Class Finder is already delivering results. In Missouri alone, more than 5,000 residents have been referred to DPP classes in the last six months. The tool gives state health departments and their partners a shared digital infrastructure for outreach, enrollment, and participant tracking, making it easier to scale impact statewide.
State health departments have access to an administrative dashboard that tracks risk survey completions, referrals by provider, class capacity and fill rates, and engagement by county. These insights support targeted outreach, smarter funding decisions, and help demonstrate public health impact.
“Before Class Finder, we had no unified way to route participants to the right class or follow up,” said Joyce Hoth, Public Health Program Supervisor at the Missouri Department of Health and Senior Services. “Now, we have visibility across the state. Our partners are supported, our outreach is smarter, and most importantly, our residents are getting connected to care that can truly change their lives.”
More states are now embedding the Class Finder into physician referral networks, Medicare and Medicaid outreach, and employer wellness programs.
