HabitNu Diabetes Prevention Program and Westmont Pharmacy Come Together to Lower the Cost of Diabetes Management for Patients in Illinois

Chicago-based HabitNu, a CDC recognized national provider of the Diabetes Prevention Program and a leader in diabetes self-management, is proud to announce its newest strategic partnership in metro Chicago with Westmont Pharmacy, located in the western suburbs of Westmont, Illinois.

HabitNu DPP is modeled after the CDC’s National Diabetes Prevention Program and offers a personalized lifestyle change service that combines a private online social network, health coaching, evidence-based curriculum, and digital tools to help individuals with prediabetes live happier and healthier lives. Over the course of the program, lifestyle coaches will work closely with participants in a group setting to help them break old habits, create new ones, set attainable goals, and keep them motivated. HabitNu will now offer its in-person DPP services to the community of Westmont Township at its local library in town and run by Westmont Pharmacy.

Diabetes has become a serious health issue in Illinois, affecting 1.2 million individuals in the State, while the number of individuals with prediabetes has grown to 3.7 million (, reflecting similar trends across the country. HabitNu is partnering with like-minded health and wellness service providers such as pharmacies, fitness clubs and community health networks to offer its CDC-recognized diabetes prevention program to individuals at risk in states with high incidents of the disease. The upshot is a highly scalable model that has resulted in proven outcomes leading to weight loss, lower A1C, and lower the cost of care.

Westmont Pharmacy has been serving the Westmont Community for more than 80 years providing retail pharmacy services and home medical equipment services. In order to create a competitive advantage and address his customer’s lifestyle needs, owner Sam Sylejmani became an early adopter of HabitNu’s program and technology.
